The file was perfect.<\/strong><\/p>\n

Every page was in order. Dates aligned. Signatures were present. Investigations were attached exactly where they should be. Anyone reviewing it would have found nothing missing.<\/p>\n

The doctor knew this because he had checked it himself \u2014 twice.<\/p>\n

When the question first came, there was no panic. No scrambling. He reached for the file with confidence, the way one does when they believe preparation will speak for itself.<\/p>\n

\u201cThis should be enough,\u201d he said.<\/strong><\/p>\n

It usually is.<\/p>\n

The problem was not what was written.
\nIt was what the file assumed would be understood.<\/p>\n

Medicine teaches doctors to document facts, not interpretations. To record observations, not atmosphere. To trust that a reasonable reader will fill in the gaps the same way they would.<\/p>\n

That assumption held for years.<\/p>\n

Until it didn\u2019t.<\/p>\n

The issue raised wasn\u2019t about negligence. It wasn\u2019t about missing information. It was about intent<\/i><\/em>\u00a0\u2014 why one option was chosen over another when both were acceptable.<\/p>\n

It couldn\u2019t.<\/p>\n

Because the decision had been made in a moment shaped by experience, context, and judgement \u2014 none of which had a dedicated line.<\/p>\n

On paper, the case looked linear.
\nIn reality, it wasn\u2019t.<\/p>\n

As questions accumulated, the doctor realised something uncomfortable.<\/p>\n

Perfection in documentation does not guarantee alignment in interpretation.<\/p>\n

The file had done exactly what it was meant to do \u2014 record events. But it hadn\u2019t anticipated the lens through which those events would later be viewed.<\/p>\n

The doctor wasn\u2019t worried about errors.
